Introducing a Range of Initiatives Designed to Improve Patient Access to Services 

 
The Jubilee Medical Practice is pioneering a range of initiatives designed to make it easier for patients and/or carers to find and access the services they need, at the right time for them, thereby avoiding unnecessary delay. Patients will benefit from the following innovations: 
 
Reception is now Patient Services: team members have received additional training to enable them to signpost patients/carers to the right service for them. Please help the team to help you by answering their questions as fully as possible. It will help to ensure you get the help you need faster; 
 
Enhanced Website Features: The website now includes a range of interactive tools which enable our patients to identify, either based on problem or symptom, the service that would be best suited to manage your problem; 
 
New Urgent Access Clinic Booking Form: which makes it clearer for our patients what problems can be managed effectively in the urgent clinic and what problems may be better addressed by an alternative service. This should cut down waiting times in the Urgent Access Clinic.  
 
Active Signposting is designed to enhance the service we deliver to our patients. Unlike in the past, where your GP was usually your first port of call, Community Healthcare is nowadays delivered by a team of professionals working at different sites across your area. We want you to be able to access the service you need without delay. This may include direct access to a mental health counsellor, without needing to wait for a GP appointment, or perhaps getting something for your cold from the local pharmacist instead of attending the Urgent Access Clinic. Active Signposting should free up more GP appointments to be used by those patients who have a problem that can only be managed by a GP.
