FRIENDS

& FAMILY TEST

We care about your experience of the service we provide; it helps us improve.

The Friends and Family Test has been created by NHS England, to help GP Surgeries identify the things we are doing well and areas that we could improve.

This is done by asking how likely you would be to recommend your Surgery to a friend or family.

All responses are anonymised. The answer to the first question is provided to NHS England, who will publish this result. The second will be used to complete and publish a practice report. Please do not include any information that may identify any individual.

Please note: The Practice can not respond to comments made via the Friends and Family Test. If you wish to receive a response to your query or cencer, please use the Complaints and Comments leaflet.
